1) ROUTINE GP FACE TO FACE APPOINTMENTS
This appointment requires you to attend the practice. Please make sure you attend the practice at the appointment time.
You will NOT be seen if you are more than 5 minuets late and the GP WILL NOT call you. The GP will only return your call if you are in the UK, if you are outside the UK you will need to make alternative provisions.
2) ROUTINE GP TELEPHONE CALL BACKS
Please make sure you leave the correct telephone number in the 'Reason' box when booking a telephone call back and that you are available to answer. The GP will only try once and can call up to 30 minuets before or after the allotted time. If you attend surgery you will not be seen. The GP will only return your call if you are in the UK, if you are outside the UK you will need to make alternative provisions.
3) MEDICATION REVIEWS WITH THE CLINICAL PHARMACIST
These are all telephone calls so please make sure you leave your correct telephone number in the reason box. Please DO NOT book anything but Medication Reviews in these appointments. Anything will not be dealt with and you will be required to re-book.
4) NURSE APPOINTMENTS FOR:
Pre-Diabetic Review with HCA - This should be attended at least 5 days prior to you Diabetic Review with the Practice Nurse. This is a face to face appointment.
Chronic Disease Reviews - To be booked for Asthma, COPD & Diabetic Reviews. If booking for a Diabetic Review please ensure that you have attended a pre-diabetic appointment with an HCA carried out at least 5 days prior to this appointment. This is a face to face appointment.
Blood Tests - Blood Tests on request from clinicians. This is a face to face appointment.
Wound Care - Dressing for wound post-op or other. Suture/clip removal after operation. This is a face to face appointment.
Cervical Smear Screening - To be booked only after a recall letter has been received. Smears WILL NOT be undertaken outside the normal recall guidelines. This is a face to face appointment.
